I understand the nature of the tattoo and the potential risks involved, including but not limited to infection, allergic reactions, and dissatisfaction with the final outcome.   I affirm that I am over the age of 18 years. If I am under 18, I have provided parental consent.   Special Request: In addition to the tattoo procedure, we kindly request that you take a moment to leave a review of your experience on our Facebook and Google pages.
